  • I am a new patient. What appointment should I book?

During this 10-minute appointment, we will review your needs goals, timeframe and any barriers to care. Following the appointment, we evaluate which treatment and provider is best suited for your needs.

We have a 24-hour cancellation policy. Any changes made 24 hours prior to an appointment may be subject to fees. This ensures that we provide our waitlisted patients enough time to attend a potential opening

  • What do I need to do before my first appointment?

We ask that you fill out your intake forms (sent via email) before arriving for your appointment. If you are unable to fill the form out in advance, please arrive 15 minutes early to do so.

Please come to your appointment with athletic clothing and/or t-shirt and shorts.

  • Do I need to put a credit card on file?

We ask to keep a credit card number in your file for the sole purpose of protecting our 24-hour cancellation policy.
